Doctor Prisoner follows Na Yi Je (Namgung Min) who is a surgeon with excellent skills and also a kind person. He was involved in a medical malpractice case that was not of his own making, which resulted in his dismissal by the hospital. He then applies for a job as a doctor in a prison, hatching a plan to flatter the business tycoons, celebrities, or sports stars imprisoned there in order to take revenge on the hospital that once ousted him. To do that, he would have to eliminate the prison's current chief medical officer and take that position.
