When Elizabeth uses the compound, an ideal, younger and more perfect version of herself appears - Sue, who is herself but in a new form. At first, Sue gives Elizabeth what she once desired: fame, limelight and public admiration. But then things quickly spiral out of control when the new self begins to have its own consciousness and demands to take over Elizabeth's real life.

The Substance is a dark metaphor for the pressure of beauty, the rejection of women in the entertainment industry, and the modern society's obsession with perfection.
